Tribal Community Health
Program Transporting Policy
It is the policy of the Tribal Community Health
Program to assist the members of the tribe by transport
ing patients to obtain their health service and to partici
pate in health presentations. In providing this service
the following priorities for transportation are:,
1. Elders who have no transportation or have transpor
tation and are incapacitated.
2. Members who have no transportation or who have
transportation but are incapacitated.
3. Children whose parents cannot transport because of
their own illness or incapacitation. It is not intended for
CHR's to become surrogate parents.
4. Members who have transportation but because of
extenuating circumstances cannot drive or it is impracti
cal to do so.
5. Groups who are attending a tribal health function
and the tribal health program has organized the trans
port. .
All transports will be assigned and scheduled by the
CHR supervisor. A transport log sheet will be main
tained listing the patient's name, purpose for transport,
destination, date and name of CHR who is providing the
transport.
