IMMUNIZATIONS
Summer is here, but all too soon it will be time for
school to start. Keep in mind that Oregon State law
provides that any child entering a certified day care
facility, pre-school, kindergarten, or first grade, or any
time thereafter, and children transferring to an Oregon
school from out of state (pre-school through 12 grade)
may face expulsion from school if they are not current
andor up to date with their immunizations.
Remember, it is your responsibility as a parent to have
your child immunized and to keep a record of those
immunizations:
NOT THE SCHOOL'S
NOT THE HEALTH DEPARTMENT'S
NOT YOUR DOCTOR'S
You will need a signed and dated copy of your child's
immunization record when heshe enters school.
Immunizations are available by appointment, Monday
through Friday, at the Grand Ronde Tribal Health .
Office.
School physicals can be scheduled during the regular
Wednesday clinic hours, 9:00 a.m. to 3:30 p.m. Be sure
to call 879-5211 or 1-800-422-0232 for an appointment.
Immunizations are also available at all County Health
Departments as well as your private doctor. Following

PARTICIPATION INSTRUCTIONS:
1. Completely fill out the official entry form and return it with the $3.00 registration fee to the Grand Ronde
Tribal Health Office before August 20, 1988 or the registration table at the event site before the event begins on
August 20, 1988.
2. If you are under 1 8 years of age, your parent or guardian must sign the waiver.
3. Race time (1 mile, 5K J3 miles) 8:00 a.m. ,
4. Tiny Tots for race time (50 yard dash) 9:30 a.m.
5. Walk for Health (1 mile, 5k 3 milesj 10:00 - 12:00 Noon.
6. You may begin and end your walk at any checkpoint between the hours of 10:00 a.m. and 12 noon.
Walk the route as many times as you wish.
