The take over of Contract Health Service will not change In terms of
health services you have received, the only difference Is you will be cal
ling the Grand Ronde Tribal Depot Office for preauthorization for health
care. The toll free number you will be calling for preauthorization is:
1-800-422-0232, (for people out of the Grand Ronde area.)
The take over procedure will mean you will continue to follow the same
procedure as before:
- Be Contract Health Service eligible (register with Contract Health
Service Program and receive approval letter.)
- Get preauthorization for health care.
- Bill your insurance, medicare, medicade before submitting your bill
to Grand Ronde Contract Health Service.
- Notify the Grand Ronde Contract Health Service Program of emergency
services received within 72 hours.
The Tribal Health Program has hired two (2) people for the Contract Health
Service Program. The Contract Health clerk is Jackie Colton, and the
Registration clerk is Karen Wilson. Jackie's position will be full-time
and Karen's position will be part-time. The clerks will be in training
for the first two weeks and will be in the office to take your calls on

The Tribal Health Program will no longer be sending out the monthly
"Health Notes" newsletter, instead the Health Program information
will be combined with the Tribal newsletter "Smoke Signals".
IMMUNIZATION ALERT 1 I !
Vacation time is ending and school will be starting again very soon. Any
child who will be entering the First grade must present a birth cert
ificate, a record of a RECENT physical examination, and an immunization
record. Oregon Law requires all children entering school to have com
pleted or be up to date with their immunization.
If your child is not up to date or has not completed his or her immuni
zations, avoid the last minute rush! Take your child to the doctor,
County Health Department, or the Grand Ronde Community Health Program's -at
the Depot office in Grand Ronde. Now!!
If your child needs a school physical, please call the Depot office for an
