Applications for the USDA Food Distribution Program arc available at the Grand Nonde Tribal Office. You may either stop by or call and ask that an application be sent to your home. Applications are also available at the USDA Food Program office located in Siletz. If you want to make an appointment for Health Services out side of direct care at Chemawa and with - in our six county service area, "please have appointments pre-author-ized by the Chemawa clinic BEFORE appointments arc made." There are some services Chemawa may not be able to pay for. In cases of KMKKCKNCIKS you must notify Chemawa within 72 hours.
