OCMUNTTV HEALTH NURSE

Duties:

1. Identification of health needs of the individual, family and oamunity.
2. Assessment of the Health Practices f the tribal population.
3. Implementation of health planning based fcn individual and family care plans and
comunity profiles. K ) t
4. Provide training and training coordination for comunity nurse assistants.
5. Evaluate impact of health activities on the individual, family and community.
6. Will make follow up hone visits on all referrals fron medical and dental clinic
with direction andor protocols. t
7. Will provide transportation of clients according to tribal policy for access to
health care and social services.
8. Will participate in health educationactivities.
9. Will provide supervision and trainin of camunity Health Assistants.
10. Will assist preparing caimunity Health Assistants work plans.
Requirements: t f

1. GIN, RN, or MPH with Caimunity experience.
2. Willingness to travel.
3. Valid Oregon drivers licenses.
4. Reliable transportation.
5. Good written and oral skills.
6. Knowledge of Indian Carnalities.
7. Willingness to train.

OUTREACH WORKER
Responsible To: Health Director
Wage: $5.00 per hour
Classification: Rill-Time (temporary) i j
Duties
1. To train (3 day maximum) to interview tribal member in the surveying methods.
2. To survey tribal members in the six county service area.
3. To assist in compiling survey data.
Requirements
1. Valid Oregon drivers license.
2. To have reliable transportation.
3. Willingness to train.
4. Ability to communicate orally in an effective manner.
5. Ability to communicate in written form effectively.
