CQMUJNITV NURSES ASSISTANT
Responsible To; Health Director
Wage; Salaried Q y,XJ-ll.(X)
Duties:
1. To observe and report health deficiencies among clients.
2. To maintain continuous cannunity contact, provide health inforniation and
available County and State service. To assist cannunity Health Nurse in
providing direct services.
3. Document and report Health needs to Health Director.
4. To provide Health information to client members for Health service.
5. To provide transportation for client member to Health service.
6. Assist Gaimunity Health Nurse in preplanning and conducting Health clinics.
7. Assist member (who request it) in making contract and direct service appointments.
Requirements:
1. Valid Oregon drivers license.
2. Reliable transportation.
3. Must be able to obtain chauffeurs license after placement.
A. Willingness to train.
5. Good cammnication skills.
6. High school or GED preferred.
7. Must be willing to accept health related training.

Duties:
1. Supervise all Health program staff.
2. Assist staff in the design of the Health program,
3. Identify unmet Health needs.
4. Develop training for Health staff.
5. Utilize information distribution.
6. Systan capable of informing membership of Health program and changes.
7. Preparation of monthly and quarterly reports. Provide some direct health care
services as appropriate.
8. Preparation of final draft of Tribal specific Health Plan.
Requirements:
1. Oregon drivers license.
2. Willingness to receive training.
3. Willingness to travel.
4. Reliable transportation.
5. Experience in an Administrative level.
6. Q1N, RN, or MPII with comunity experience.
7. Good writing skills.
8. Knowledge of Indian Tribes.
