Eligibility
We provide most services free of charge to eligible Indian people and
their dependents. Some dental work may require your participation in
payment for laboratory and material fees. To be eligible for health
care services at Chemawa, you must be a member of a federally recognized
Indian tribe or Alaska Native group. If you a a new applicant for ser
vices, we need to have evidence of enrollment submitted with your appli
cation. Usually the best way to obtain this is to contact your tribe or
the Bureau of Indian Affairs office in the area you were born. Please
call 370-4204 or 1-800-452-7823 if you have any further questions.
Medical Clinic
We are open from 8:00 a.m. to 5:00 p.m. every weekday except Wednesdays
and Fridays when we close at 1:00 p.m.
Appointments are available for diagnosis, treatment, well-child care, immu
nizations, prenatal and post-delivery care, family planning (birth control),
minor surgical procedures, office orthopedics, and preventive care. The
clinic is well equipped and able to provide a wider variety of services than
most private physician's offices. If necessary you may be referred to special
ists with the Chemawa physician responsible for coordinating your care and help
ing you get through the medical system.
We cooperate with the Medicare and Medicaid programs. -If you participate-in
either of these programs, show us your eligibility card when you come in for
a visit. We can obtain additional resources from these programs that will help
us maintain the building and acquire needed equipment.
Dental Clinic
An experienced dental team is available to treat your tooth and gum problems
as well as offer up-to-date information on how to prevent dental disease. We
want to establish a partnership so that we can provide comprehensive dental
treatment and you can achieve and maintain good dental health.
Except for emergencies, dental services are available on an appointment basis.
Clinic hours are from 8:00 a.m. to 4:00 p.m., Monday through Friday except
Wednesdays, when we close at 1:00 p.m. If you are unable to make a scheduled
appointment, please notify the dental clinic at least 48 hours in advance.
Appointments for special services such as dentures, partials, crowns and
bridgework are given only as time and funding allows
Environmental Health
The Sanitarian at the Chemawa Indian Health Center provides consultation and
training concerning environmental health services. Upon request the sanitarian
may provide advice and technical assistance in the areas of water quality, wast
management, food sanitation, air quality, institutional health and safety, . vect
control, recreational health and safety, and community injury control.
