Diabetes Self Care sessions for 2013
T he D iabetes Self Care
Sessions are held on the sec­
ond and fourth Tuesdays o f
each, month, from 11 a.m. to
noon, with light Lunch after­
ward.
The sessions are presented
by Jeri Kollen, RN, who is a
Certified Diabetes Educator
with the IHS Model Diabe­
tes Program Education; and
Linda Portef, Warm Springs
Tribal Nutritionist.
T he D iabetes Self Cafe
sessions are held in the
kitchen conference room at
the Warm Spring Health and
Wellness Center.
A ttending four different
sessions, and a follow-up visit
th ree to six m onths later,
com pletes the H O P E p ro ­
gram.
