Case Manager at Irrigon
Medical Clinic. Will also see
patients at Pioneer Memorial
Clinic in Heppner. The Be-
havioral Health Case Man-
ager provides a wide range
of clinical and administrative
services to improve client
care within the primary care
setting of our clinics. The
Case Manager works closely
with the Licensed Clinic So-
cial Worker and medical pro-
viders, and collaborates in the
management of a caseload of
patients with varying medical
concerns, mental illnesses
and addiction issues. Must
possess a Bachelor’s degree
in a behavioral science field

university with minimum two
