The Hermiston School District (“District”) invites sealed written
proposals for health care services to be provided in a dedicated
Wellness Center located in the Hermiston High School. The scope
of services to be provided include, but are not limited to, well visits,
screenings, referrals, vaccinations/immunizations, lab services,
and insurance counseling for District students and District faculty
(employed half-time or more) and their Oregon Educators Benefits
Board insurance-covered dependents.
Prospective proposers may obtain a full copy of the RFP, including
the terms and conditions of the Agreement to provide the request-
