The Board of Inquiry finds that
a passenger train, near Mayo,
Aug. 22. while rounding a fifteen
degree curve, on a three ntr itnt
ascending grade, at a sieed of
fifteen miles per hour, had the
forward wheels on rear truck of
last coach derailed, running a
distance of 210 feet from print
of derailment: train being
brought to a stop by application
of conductor's valve by rear
brakeman.
The examination of thA iminrv.
ment involved conclusively dis
closed that this derailment r
suited from Wheel No. 4 shifting
us wneel seat 138 inches,
which caused it to mount rail on
outside of curve. Kemoval of
wheel from its axle showed that
it had not been thoroughly and
firmly applied; which defect.
however, was not cetectable un
der ordinary inspection;
