through September 22, 2013 at 11:59pm.

Applications will be selected and ordered using a
random lottery system. The day and time an
application is submitted has no impact on
whether an application will be selected for the
waitlists.
Paper applications are available by mailing a
written request to: HACC, Attn: Waitlist Opening,
PO Box 1510, Oregon City, OR 97045, before the
waitlists open. Completed paper applications must
be mailed to HACC at the address above and post­
marked between September 16th and September
22nd, 2013, to be accepted.

services such as engagement and assessment,
intervention and treatment, training and docu­
