Standard Oregon Review Board prequalification form s, fo r the
class o f work for which C ontractor is su bm itting a bid, at least
ten (10) days prior to bid opening. P requalification form s may
be obtained from the Engineer and should be su bm itted to the
Owner via the Engineer (HGE INC., A rchitects, Engineers, Sur­
veyors & Planners - Coos Bay). If a prequalification form is on
file with the Engineer and is not over one year old, refiling is not
necessary unless there has been a su b s ta n tia l change in
Contractor’s status.
Work included in th is project is fo r public work subject to the
Oregon Prevailing Wage Rate Laws (ORS 2 7 9 .3 4 8 to 2 7 9 .3 8 0 ).
Bidders attention is directed to the requirem ents o f employment
and m inim um wage rates to be paid. No bid will be considered
or received by the LaPine Water D istrict unless the bid contains
a sta te m e n t by the bidder as p a rt o f its bid th a t the provisions
required by ORS 2 7 9 .3 5 0 are to be com plied with. Require­
m ents for Bidders and Contractors under th is Order are detailed
in the C ontract Documents.
Each bid m u st contain a sta te m e nt as to w hether the Bidder is
a R esident Bidder as defined in ORS 2 7 9 .0 2 9 .
Successful Bidder m ust be registered w ith the Construction
C ontractor’s Board as required by ORS 6 7 1 .5 3 0 . Bidder is not
required to be licensed under ORS 4 6 8 A .7 2 0 .
No bid will be considered unless fully com pleted in the m anner
provided in the "Instruction to B idders” , upon the Bid Form (Pro­
posal) provided by the Engineer, and accom panied by a certified
check or Bid Bond executed in favor o f LaPine W ater D istrict in
an am ount not less than ten percent (10%) o f th e to ta l am ount
o f the bid. Bid Bond is to be fo rfe ite d as a fixed and liquidated
damage should the successful Bidder neglect or refuse to enter
into a contract and provide suitable bonds to ensure the faithful
perform ance and payment o f the work (in the event the contract
is awarded such Bidder). Funds for the im provem ents are avail­
able, and m onthly paym ents w ill be m ade by check.
C ontracts awarded fo r th is project w ill be financed w ith a grant
and loan from the Rural Development branch o f the United States
D epartm ent o f Agriculture. Prior to any C ontract Award, com pli­
ance with all USDA/Rural D evelopm ent requirem ents w ill be
verified. Similarly, all project paym ents, change orders, and as­
sociated paperwork will be subject to USDA/Rural Development
review and approval prior to becom ing effective.
Bidders will be required to com ply w ith the provisions o f the
President’ s Executive Order No. 1 1 2 4 6 concerning Equal Em­
ployment O pportunity (EEO), including all issued am endm ents

LaPine W ater D istrict may reject any bid not in com pliance with
all prescribed public bidding procedures and requirem ents, and
may reject for good cause any and all bids upon a finding o f the
