Metro, located at 600 NE Grand Avenue, hereby requests
written proposals for the following services for the Regional
Environmental Management Department.
Medical Surveillance Services Metro is seeking proposals
I from qualified firms to perform medical surveillance exam ina­
tions and records administration for up to sixty (60) Hazardous
Waste employees. Proposers are invited to contact Sally
Koch at (503) 797-1853 to schedule a walk-through of the
Metro South and Metro Central HHW facilities. Metro is
! seeking to contract with the successful com pany for a period
| of three years with a two year extension at M etro’s request.
Proposals must be delivered to Metro, 600 NE Grand Avenue,
Portland, Oregon 97232-2736, Regional environmental M an­
agement Department, no laterthan 5:00 PM, Tuesday, Octo­
ber 15, 1996.
Proposals must be submitted in sealed envelopes which
clearly identify the services as stated in the RFP. That RFP
represents the most definitive statement Metro will make and
is available at the above address from 8:30 AM to 4:30 PM,
Monday-Friday or by calling (503) 797-1650.
All proposals must conform to the RFP format, answer all
questions and be complete including the use of any required
forms. Metro may reject any proposal not in compliance with
the prescribed RFP requirements and any or all proposals if
[such action is deemed in the public interest.
