deadline for Statements of Qualifications on its flexible services
for pavem ent design Request for Qualifications, the new
deadline for Statements of Qualifications is 5:00 p.m. Novem­
ber 3, 1992.
The Oregon Department of Transportation, is seeking qualifi­
cations from qualified consultants to assist the Department in
developing pavement designs during peak workload periods.

Unless otherwise stated in the individual proposals listed
herein, no proposal or bid will be considered unless accom pa­
nied by a bid surety for an amount not less than ten percent
(10%) of the aggregate amount of the proposal.
The City encourages bidding by MBEs and F BEs and will assist
such firms to understand and participate in formal bidding
Principal items of work consist of the following:
Schedule A - W astewater Treatment Plant Upgrade: The work
includes demolition of existing piping, equipment and structures;
m odifications of the influent pump station and headworks to
handle 6.0 mgd; and construction of asecondary clarifier, gravity
sand filter system and dechlorination system.
Schedule B - Relief Sewer Improvements: The work includes
constructing approximately 560 feet of 10-inch diam eter and
1350 feet of 8-inch diameter sanitary sewer, including manholes
and related appurtenances. The work includes two bored sec­
tions totaling approximately 125 lineal feet.
Bids shall be based on the contract lump sum for Schedule A
work and unit prices for Schedule B w ork as stated in the
Contractors Bid Form. The Contractor may submit a bid for either
or both schedules. The City reserves the right to award each
schedule separately.
W ork shall be completed within 240 calendar days for Schedule
"A" and 120 calendar days for Schedule "B” after the date set
forth in the Notice to Proceed. Notice to Proceed will be given in
March. 1993.
No bid will be considered unless fully complete in the manner
provided in the “ Instructions to Bidders” upon the bid form
provided.
Each bid shall be accompanied by a bid bond or a certified check
or cashiers check made payable to the City of Sweet Home in an
amount not less than 10% of the amount of the bid. The bid bond/
check shall be given as a guarantee that the successful bidder
will execute the contract if it is awarded in conformity with the
contract documents and shall provide surety bond or bonds as
specified therein within 15 days after notification of Award of
Contract to the bidder.
