the project specified herein.

W ork Description: Remove and replace office carpet at 360
Church Street S.E., Salem, Oregon.
Bids must be submitted on the bid forms furnished to bidders. Bid
forms shall be submitted in a sealed envelope plainly marked
“ BID ON CARPET REPLACEMENT - HOUSING AUTHORITY
ADMINISTRATION OFFICE, BID NO. 3985,” and show the
name and business address of the bidder.
The Contract will be awarded on the basis of the base bid unless
it overruns the amount budgeted for this work. In the case of such
overrun, deductible alternates will be taken by the SHA in num eri­
cal order as they are listed to the extent necessary to come within
the budgeted total. If there are no deductible alternates or taking
all deductible alternates the resultant still exceeds the budgeted
amount, the SHA reserves the right to reject all bids, or to secure
additional funds necessary to awarding the Contract.
A surety bond, cashier’s check, or certified check of the bidder in
the am ount of ten percent (10%) of the bid must be attached to
each proposal as bid security. Unsuccessful bidders will have
their security refunded to them when the contract has been
awarded.
The successful Bidder will be required to furnish and pay for sat­
isfactory Performance and Payment Bond, in the full amount of
the Contract.
Attention is called to the provisions for equal employment oppor­
tunity, insurance requirements, and the payment of not less than
the minimum salaries and wages as set forth in the Specifications
must be paid on this project.
As permitted by state law and required by the U.S. Department of
Housing and Urban Development:
a. Retainage is 10 percent not 5 percent,
b. Interest will not accrue on retainage, and
c. Bonds or securities may not be substituted as an alternate form
of retainage.
The Salem Housing Authority reserves the right to reject any or all
bids, to waive formalities in the bidding and of postponing the
award of the contract for thirty (30) days. Prices bid shall be firm
for a period of 30 days after the closing date.
No Bidder may withdraw his bid within thirty days after the date of
bid opening.
NOTE: This project is federally funded through the Department of
Housing and Urban Development and all requirements of that
agency pertaining to bidding and contract performance shall be
strictly adhered to.

The Oregon State Office of Medical assistance (OMAP) invites
interested health care providers to submit Application for Prepaid
Managed Health Care for AFDC clients living in the above-
named counties.
OMAP will contract with qualified Physician Care Organizations
(PCO) to provide prepaid managed health care, including physi­
cian, pathology, radiology, and case management services.
Applications for providing additional medical services (not includ­
ing hospitalization), such as pharmacy, physical therapy, dental,
and chiropractic may also be offered.
If sufficient service capacity exists, enrollment in a PCO may be
mandatory for AFDC clients living in specific ZIP codes or other
geographic areas.
Responses to the RFA must state whether the responder is a
resident as defined under ORS 279.029.
