Request For Statement Of
Qualifications
To Providee Consulting Services
For The Parks And Recreation Division

City Of Gresham, Oregon
The City of Gresham plans to retain one consultant to provide
professional design and construction administration services for
the developm ent of three neighborhood parks in 1991-92: Hol-
lybrook Park, 2.4 Acres (CIP #7025), Kirk Park Phase 1,5.7 Acres
(CIP #7026), and North Gresham Park Phase 1,12.79 Acres (CIP
#7028). Consultant services will include public meetings, design,
construction documents, City Community Service Development
and Building Permits, bidding, inspections and contract adm ini­
stration services for construction of the following improvements:
Tree pruning and removal, site clearing, grading and drainage,
irrigation systems, seeded lawns, baseball field, soccer fields,
drinking fountain, play equipment areas, landscaping, asphalt
paths, site lighting, horseshoe pits, picnic areas and site furnish­
ings. The total budget available for these projects is $435,000,
which includes all consultant services and construction.
Interested consulting firms shall submit to the Gresham Parks &
Recreation Division office, Attention: Phil Kidby, Parks Planning
& Development Program, 1333 N. W. Eastman Parkway, Gresham,
OR 97030, by 2:00 P.M., Pacific Daylight Time, Friday, October
4, 1991, a 5-page Statement Of Qualifications which shall in­
clude:
1. Firm Info rm ation. Firm name, address, telephone number,
contact person, year established, types of services qualified to
perform, and number of personnel of the firm.
2. Q u a lific a tio n s of the Firm . A summary of projects completed
by the firm which are comparable to the services required for the
im provements proposed including name and location of the proj­
ect, date of the project, name and telephone number of owner
representative, and name of consultant’s manager on the proj­
ect.
3. Q u a lific a tio n s of P ro je ct Personnel. The name, qualifica­
tions, and specific work experience of key personnel proposed
for this project. An estimate as to the percentage of time these
personnel would be available during the various phases of the
project shall be indicated.
These qualifications should indicate related projects worked on
and include the name of the project, dates, and specific items
worked on. Also, submit a list of alternative personnel available
to work on the project should any of the key personnel indicated
be unavailable to work on the project should any of the key

Please submit qualifications and experience of alternative proj­
ect personnel. Resumes of other important project staff may also
be submitted.
The statement of qualifications will be evaluated on the following
criteria:
1. Firm experience and quality of prior work.
2. Qualifications and experience of team members to be as­
signed to this project.
3. Past performance on similar projects.
Based upon the statements of qualifications, three to four of the
most qualified firms will be selected to submit detailed proposals.

FY 91/92 U.S. Bank Zoolights Festival: Light Installation Con­
tract at the Metro Washington Park Zoo. Sealed bids must be de­

Metro has made a strong commitment to provide maximum op­

portions of this project, will be required to meet the goals or dem ­
onstrate good faith efforts to meet Metro’s Disadvantged Busi­
ness program goals.
Bids must be accompanied by a certified check, cashier’s check

surety authorized to issue such bonds in the state where the
work is located, in the amount of $100. This bid security shall be
given a guarantee that the Bidder will not withdraw his/her Bid for
a period of sixty (60) days after bid opening, and that if awarded
the Contract, the successful Bidder will execute the attached

Metro may reject any bid not in compliance with all prescribed
public bidding procedures and requirements and may reject for
good cause any or all bids upon a finding of Metro that it is in the

hemiah Housing Opportunity Program.
The Northeast Community Development Corporation (NECDC),
(the Agency) requests Contractors Statement of Qualifications
for construction of new houses and extensive renovation of ex­
isting older homes. A total of 250 houses will be constructed or
renovated under the Federal Nehemiah Housing Opportunity
Program in the King, Humboldt, Vernon, and Boise neighbor­
hoods of Northeast Portland. Davis-Bacon and Oregon Prevail­
ing Wage Rates do not apply.
Bids will be requested beginning in October, 1991 on eight proto­
type model units. Main construction/renovation program will
begin early in 1992 at the rate of 10 construction starts per
month. Bids will be solicited from contractors prequalified by the

The City of Salem will receive sealed bids at the office of the City
Recorder, City Hall, Room 205, Salem, Oregon, until but not
a fte r 11:00 a.m., October 2,1991, at which time said bids will be
publicly opened and read aloud in the City Council Chambers,
Room 240,555 Liberty Street S.E., Salem, Oregon for the project
specified herein.
The proposed work consists of: approxim ately 2,325 linear feet of
8-inch PVC, 245 linear feet of 8-inch ductile iron, 97 service con­
nections, 9 manholes, with other appurtenance work.
Plans, specifications and other bid docum ents may be inspected
and obtained at the office of the City of Salem, Department of
Public Works, 555 Liberty Street S.E., Room 325, Salem, Oregon
(503) 588-6211). The bidder shall not file the book of “ Standard
Construction Specifications” with his bid.
Bids must be submitted on the proposal form s furnished to the
bidders. Proposals shall be submitted in a sealed envelope
plainly marked "Bid on the replacement of an 8-inch sanitary
sewer in Gerth Avenue to Patterson Avenue NW - Bid #3962”
and show the name and business address of the bidder.
Any objections to or comments upon the bid specifica­
tions must be submitted in writing to the Department of
Public Works, Room 325, 555 Liberty Street, SE, Salem
OR 97301. They must be received no later than ten (10)
working dayss before the bid opening date.
A surety bond, cashier's check or certified check of the bidder in
the amount of ten percent (10%) of the bid must be attached to
each proposal as security. Unsuccessful bidders will have their
security refunded to them when the contract has been awarded.
No bids, exceeding $10,000, will be received or considered by the
City of Salem or any of its officers unless the bid contains a
statement by the bidder (by signing his proposal) that the provi­
sions of ORS 279.350 shall be included in his contract.
Every contractor or subcontractor is required to post the appli­
cable Prevailing W age Rates (PWR) in a conspicuous and acces­
sible place in or about the work-site for the duration of the job.
Contractors and subcontractors who intentionally fail to post the
PWR can be made ineligible to receive any public works contract
for up to three years.
RECIPROCAL PREFERENCE LAW: In compliance with ORS
279.029, bidders must state on their proposal whether they are a
resident or nonresident bidder. Proposals that fail to provide this
information will be considered nonresponsive.
The City of Salem reserves the right to reject any or all bids, to
waive formalities and of postponing the award of the contract for
thirty (30) days.
Prices quoted shall be firm for a period of 30 days after closing
date.
The attention of bidders is directed to the provisions of Chapter
97, Salem Revised Code, concerning unlawful employment prac­
tices. Violation of such provisions shall be grounds for immediate
termination of this contract without recourse by the contractor.
