The individual in these Portland, Eugene and Medford positions will
conduct physical audits of books, records and operations on insured
employers; provide documented support of premium base renumeralion,
Workers’ Compensation classification and any excluded renumeration.
Outside travel and occasional overnight travel required. Must manage
workloads and schedules that necessitate early morning travel or later
evening travel.
Applicants must have a two year college degree, preferably four years in in
surance, Accounting and/or Business. One year in related business/
financial environment with demonstrated work abilities in handling and
communicating rules and procedures.

The individual in this Salem position will maintain an efficient, motivated,
trained technical unit to resolve claims properly and fairly in accordance
with corporate claims policy and procedure, rules, laws, etc.
Applicants must have a minimum of five years of workers’ compensation of
insurance related experience in positions of increasing responsibility;
college degree and/or insurance related education. Supervisory
exp[experience preferred.

The individual in this Portland position will be responsible for performing
industrial hygiene consultation for insured to reduce the incidence of
occupational related disease; provide informational training on indus
trial hygiene subjects to insured and SAIF staff; and investigate occupa
tional disease claims.
Applicant must have a Bachelor of Science degree in industrial hygiene or
related science and two years of experience in an industrial hygiene field.
Advanced degrees and additional industrial hygiene experience strongly
preferred. Individuals must have a current knowledge of the field of
industrial hygiene including toxicology, air monitoring, noise measure
ment, ventilation, calibration and operation of industrial instrumenta
tion.

FOR CONSTRUCTTON OF
GASTON FORCE MAIN
FOR
UNIFIED SEWERAGE AGENCY
OF WASHINGTON COUNTY, OREGON
Sealed proposals for the construction of the Gaston Force Main for the
Unified Sewerage Agency of Washington County, Oregon, will be received
by the Unified Sewerage Agency at 150 North First Avenue, Hillsboro,
Oregn 97124, until 2:00 PM, local time, on the 7th day of July, 1989, and
then will be publicly opened and read.
The work contemplated consists of construction of the following:
Schedule A: Construction of approximately 10,600 lineal feet of 8-inch
diameter PVC sewage force main, and related piping and appurten-
cances and the replacement of two vertical non-clog centrifugal sewage
pumps, associated electrical modifications, concrete pump base and
related piping in an existing pump station.
Schedule B: Construction of approximately 10,500 feet of 8-inch di
ameter PVC sewage force main, and related piping and appurtenances.
Schedule C: Construction of approximately 14,300 lineal feetof 8-inch
diameter PVC sewage force main and related piping and appurten-
cances, yard piping and valving at an existing sewage treatment plant,
a force main air injection system consisting of two air compressors, a
pre-engineered steel compressor building, associated HVAC equip
ment, associated electrical work, approximately 14,800 lineal feet of 2-
inch diameter PVC air pipe, approximately 900 lineal feet of 2-inch
diameter PVC chlorine solution pipe, and related piping and appurte
nances.
The contract documents may be examined at the office of MURRAY,
SMITH & ASSOCIATES, INC., Engineers/Planners, 12 1 S W Salmon Suite
1110, Portland, Oregon 97204, or at the office of the Unified Sewerage
Agency 150 N. First Avenue, Hillsboro, Oregon 97124. Copies of the
contract documents may be obtained at the office of MURRAY, SMITH &
ASSOCIATES, INC. A non-refundable fee of $25.00 is required for each set
of documents.
Contractors must be qualified in accordance with the applicable parts of
ORS 279 in order to enter into a contract with the Owner for public work in
Oregon. Bidders who are not currently prcqualified by the Owner to perform
the type and size of work contemplated here in shall submit a completely
executed Contractor’s Prequalification Application to the Owner prior to, or
at the time of bid opening. Bidders who are in doubt as to their current status
with the Owner regarding prequalification are encouraged to contact the
Owner to determine prequalification status. Prequalification application
forms may be obtained at the State of Oregon Department of Transportation,
Salem, Oregon 97310. The owner will investigate and determine the
qualifications of the apparent low bidder prior to awarding a contract.
Financial assistance for this project is provided by a Grant from the
United States Environmental Protection Agency administered by the Ore
gon Department of Environmental Quality. Attention is directed to the
Federal and State requirements regarding Equal Employment Opportunity,
Executive Order 11246 and Executive Order 11375, Davis-Bacon Wage
Rates and ORS 279 requirements. The successful contractor shall provide a
minimum of 8% Minority Business Enterprise involvement and a minimum
4% Women Business Enterprise participation.
Any contract or contracts awarded under this invitation for bids are
expected to be funded in part by a grant from the U.S. Environmental
Protection Agency. Neither the United States nor any of its departments,
agencies or employees is or will be a party to this invitation for bids or any
resulting contract. This procurement will be subject to regulations con
tained in 40 CFR 35.936, 35.938, and 35.939.
The prime contractor, subcontractors, and lower-teir subcontractors
shall assure the Unified Sewerage Agency that no subagreement (contract
or subcontract) for construction, alternation, or repair of a public building
or public work will be awarded to: (1) A Japanese citizen or National, (2) a
firm controlled directly or indircdtly by Japanese citizens or Nationals, or
(3) a supplier of any product if more than fifty percent (50%) of the total cost
of the product is allocable to production or manufacture in Japan. All bids
or proposals from such firms or suppliers shall be deemed nonresponsive
and will be rejected.
Qualified contractors are hereby notified that a pre-bid conference
regarding Federal and State requirement for this project will be held on
Thursday, June 22,1989 at 1:00 PM Pacific Standard time in the 3rd floor
conference room of the Unified Sewerage Agency, 150 N. First
Avenue, Hillsboro, Oregon 97124.
No proposal will be received or considered by the Agency unless the
proposal form contains, or is accompanied by, a statement by the bidder as
a part of the proposal that the provisions required by ORS 279.350
pertaining to prevailing wages shall be included in the contract. No
conditioned bid proposals will be considered or accepted by the Unified
Sewerage Agency/
Proposals must be submitted on the prescribed forms and must be
accompanied by a certified check or bid bond executed in favor of the
Unified Sewerage Agency in an amount not less than 5 percent of the
amount bid. The successful bidder will be required to furnish the necessary
additional bond(s) for faithful performance of the contract as prescribed in
the contract documents.
The Unified Sewerage Agency reserves the right to reject any and all
bids, to waive informalities, or to accept any bid or combination thereof
which appears to serve the best interest of the Agency.
LEGAL NOTICE
The Department of Housing and Urban Development will accept applica
tions from nonprofit organizations for rental or cooperative housing under
the Section 202 Direct Loan Program for Handicapped People. Appropriate
instructions, forms, and other program information arc contained in an Ap
