High Risk Triage Coordinator
***This position is based in Seaside, OR, option for partial
telecommuting from another OR/ SW WA Coastal town***
Works closely with the Population Health team to coordinate care using a
trauma-informed and member-centric approach. Takes into consideration
the global health of an individual given their physical, social, mental and
dental health needs and coordinates timely access and continuity of care to
promote positive health outcomes. The overall aim is to improve quality of
care, coordinate care across the continuum, reduce hospital readmissions,
and increase access to advanced illness care. Responsible for identifying
and triaging members and assignment of appropriate referrals. Core
functions of the position include addressing membersʼ care coordination
needs, identifying appropriate resources to engage, and providing support
to members across the risk spectrum as needed.
Requirements: Minimum 3 years of directly related experience working with
vulnerable populations in a healthcare setting, social service agency,
outpatient primary care clinic, hospital, or health insurance plan. Experience
should include working with consumers of Medicaid and Medicare
healthcare services and an understanding of the psychosocial and medical
barriers encountered by this population. This position may require some
day trips and may require overnight travel.
