    SUMMARY OF SILETZ TRIBAL PROGRAMS (cont.)
JOHNSON O ’MALLEY PROGRAM
The Johnson O ’Malley Program serves students, (ages three through 12th grade) of all
Federally recognized Tribes. Nature of Program Service: Supplemental Educational
program designed primarily to meet the needs of eligible students.
Develop good communications between school personal, students and parents.
Implement cultural activities to better the awareness of students interested in
cultural Arts and Crafts.
Field Trips to various places of interest for the Indian students.
The Tutor/Counselor program is to serve the Educational needs of Indian students.
Parental Cost Component assists in buying supplies for students if parents income
threatens to deprive them of school activities.
C O M U N I TY HEALTH REPRESENTATIVE
The Community Health Representative will be glad to assist you in Health p l a n s for
your family, as a source of information to help you understand and receive the
health care available to you as a member of the ¿Siletz Tribe. Dental, eye care and
pharmacy services also are available and the CHR can assist you to obtain them.
Routine care is now available. Pre-Authorization is required for all services,
except emergencies. For Authorization call your toll free number 1-800-452-7825.
Report your emergency visits within 72 hours. All other resources must be
utilized before applying for IHS services such as Insurance, Medicare, medicaid
and all Public assistance.
Medical:
You may receive contract care from your local physicians and hospital.
EMPLOYMENT
The Siletz and Salem Area Offices also provide employment services under the
Comprehensive Employment and Training Act. These servic&s include public Service
employment, on-the-job-training, direct placements, education and support services.
This program is available to any eligible Indian person.
