    PHS - INDIAN HEALTH CENTER - CHEMAWA INDIAN SCHOOL
DENTAL FUNDS are now available for routine dental care, upper and lower dentures and
lower denture relines. Other dental work must be considered on a case by case basis.
Call 1-800-452-7823 and ask for the Contract Health Service clerk when you make an
appointment with the dentist. When you have your DENTAL APPOINTMENT, THE DENTIST IS
TO DO ONLY AN EXAM AND X-RAY, THEN THE DENTIST WILL SEND OUR FORM TO OREGON DENTAL SERVICE
for pre-authorization. You will have to wait a few weeks to have the actual work done
EXCEPT FOR EMERGENCY CARE.
PHS - INDIAN HEALTH CENTER - CHEMAWA INDIAN SCHOOL has hired two new employees. Both
ladies are Siletz Indians. Lorraine Hesketh is the new Contract Health Service clerk and
will start working March 17; Janet Russell is hired as a clerk/typist starting her
employment March 24, 1980. Lorraine will be taking Adrienne Hall’s place at Chemawa.
Adrienne is moving to Portland.
MEDICAL CARE REMINDERS: Please call 1-800-452-7823 before your appointments'; for< medical
care for pre-authorization or INDIAN HEALTH CANNOT PAY THE B T T .T ., Let them know if you
are referred by your doctor to another doctor or have other x-rays, lab work, etc. Call
this same telephone number before any hospitalizatipn unless it is an emergency and then
call the Clinic within 3 days or 72 hours.
If you have Insurance or Medicare still call regarding appointments and send IHS (Chemawa)
a copy of your explanation of benefits from your insurance co. after the insurance pays,
so IHS can pay the remainder. Give your Doctors office your name for billing and the
following address: PHS - Indian Health Center - Chemawa, 3750 Hazelgreen Rd., N.E.,
Salem, Oregon 97303.
Please call us if you are on Welfare and we will explain how our benefits work. Eye exams
are okayed yearly and you must go to DR. Michel Gaynor in Salem or Newport Vision Center
in Newport.
Special consideration to see another Doctor must be requested. Call us first
before making an appointment with either of the above Doctors.
If you live within 25 miles of Salem please come to our Clinic for Medical care. We now
have x-ray and pharmacy services. Call for an appointment first and check with our pharmacy
before coming to make sure we have the medication.
NORTHWEST PORTLAND AREA INDIAN HEALTH BOARD
NORTHWEST PORTLAND AREA INDIAN HEALTH BOARD STUDENT SUMMER PROGRAM 1980 - June 14 -
July 26 1980. The Northwest Portland Area Indian Health Board, Health Careers Opportu­
nity Program will be facilitating a SIX-WEEK SUMMER PROGRAM FROM June 14 to July 26, 1980,
TOR NATIVE AMERICAN HIGH SCHOOL SENIORS AND COLLEGE FRESHMEN AT PACIFIC UNIVERSITY, FOREST
GROVE, OREGON.
The purpose of the summer program is to prepare Native American students for their up­
coming college curriculum and college environment. Mini-courses will focus on increasing
math, chemistry, biology and english skills.
ELIGIBILITY:
1. This program is geared towards students who are presently high school
seniors or college freshmen and wish to pursue a health career. 2. Applicants should
have completed a minimum of one year of math and one science course.
3. Applicant must
be of Native American or Alaskan Native descent. 4. Applicants should be 18 years or
older.. (SOME EXCEPTIONS ON AN INDIVIDUAL BASIS MAY BE MADE TO THE ABOVE)
STUDENT EX­
PENSES: 1. Each participant will be provided with room and board at Pacific University.
2. Round-trip mileage (15^ per mile) to Forest Grove, Oregon will be provided to each
participant.
3. Each student will i 'eive a $350.00 stipend for their attendance and
participation.
