Look for our column printed on the fourth Wednesday edition of the ‘Illinois Valley News.’ Question: I have a family member in the hospital. Can they get help from hospice? Answer: Yes. Your loved one’s physician can ask for a Hospice consul- tation even while the patient is in the hospital. Or you can call your Hospice of choice directly to obtain information about what the Hospice benefit in- cludes. It is important to the patient, family, the physician, hospital staff, and Hospice that “seamless” care be available. This means that the move from the hospital to home be as smooth as possible, with necessary equipment and medications in place, and home and caregivers prepared. If a life-threatening illness is involved, if the physician has indicated that there is little more that advanced treatments can offer, and if the patient would bene- fit from having comfort care and home visits from Hospice professionals, then perhaps early planning for the trip home should include Hospice. Hospice offers assistance in placing medical equip- ment into the home if needed, medica- tion coordination with the physician, ongoing education for home caregivers, assistance with obtaining caregivers if necessary, counseling for the patient or family in dealing with difficult changes in life and feelings, volunteers, and nurs- ing and home health aid services in the home. If you or your loved one are interested in Hospice, please ask your physician, or you may inquire about Hospice by calling your Hospice of choice. If the patient is in the hospital, you may also inquire through the hospital discharge planner.
