Wishing the Siletz Community Health Clinic a Happy Birthday Patricia Canning and Heather Claffey By: Savannah Worman The Siletz Community Health Clinic (SCHC) celebrated its 35 th birthday with community members and staff. Birthday festivities were in full swing with cake, raffle prizes, and lively conversations. The Confederated Tribes of Siletz Indians and Indian Health Services came together with a vision for SCHC 3 decades ago, and the clinic located on Swan Avenue in downtown Siletz opened its doors on Feb. 9, 1991. The clinic on Swan Avenue housed Jacob Mathis, Kathleen Alison-Meyers, Selene Rilatos Medical, Laboratory, Radiology, Pharmacy, Dental, Optometry, Com- munity Health Advocate, Nutrition Education, Tobacco Prevention Edu- cation Program Coordinator, Mental Health Care and Patient Accounts. During this time Contract Health Services (CHS) and the Alcohol & Drug (A&D) program were located in a separate location on Old River Road due to the lack of office space. SCHC was also able to expand by adding the Tillicum Fitness Center located on Buford Avenue. Pequot Prescription Reimbursements If you have paid out of pocket for your prescriptions, Pequot can reimburse you up to $500 for the calendar year. Please do not wait until the end of the year to send in receipts as there is no guarantee how far back Pequot will do reimbursements. To start the process for reim- bursement, please send in your receipts quarterly to Purchase and Referred Care (PRC) via mail or email. It takes 4-8 weeks to receive Pequot reimbursements. When SCHC relocated to its cur- rent building on Gwee-Shut Road, in 2010, it was able to consolidate the external programs such as hous- ing CHS and A&D in the main clinic facility. The clinic has been able to sustain services, expand services and add new programs to continue outreach to our members. Some of these programs include, early intervention/peer recovery services, non-emergency medical transporta- tion, the CARE Program, and the farm property. The clinic has been able to sustain itself for so long by running on its core standards. Providing high qual- ity health services, treating patients with respect, consideration, and dig- nity. Promoting the improvement of professional skill and quality of per- formance, using protected electronic records systems, and finally by provid- ing a safe and sanitary environment. SCHC staff are excited to have spent this milestone with the community and look forward to many more birth- days at the clinic.
