Rehabilitation Program
Formerly Over-Income Rehabilitation Grant
The Over-Income Rehabilitation Construction program funds over-income families
with a $10,000 grant for construction rehabilitation on the primary residence they own.
The over-income grant is for eligible applicants and is limited to one grant per
person/address/household. If you have already received a General Welfare Exclusion
Housing Rehabilitation Grant, you are not eligible to apply.
If you have received services for the following programs, you are not eligible to
apply for the General Welfare Exclusion Housing Rehabilitation Grant for five years:
Down Payment Assistance, NAHASDA Rehabilitation Program, NAHASDA Elders
Replacement Program, NAHASDA Rehabilitation Program for Elder Tribal Members,
NAHASDA Rehabilitation Program for Disabled Tribal Members, Homeownership
Program/Mutual Help and BIA Self Governance.
Rehabilitation refers to repairs or renovations to the home, but does not include
appliances, saunas, hot tubs, swimming pools, driveways, fences, etc.
Over-income refers to Tribal members whose income exceeds the income limits set
by the Native American Housing Assistance and Self Determination Act (NAHASDA).

Applications, including proof of current annual income, enrollment and homeown-
ership with the Tribal member’s name on it, must be submitted to the Housing office
by Tuesday, July 31, 2018, at 4:30 p.m. Applications received after that date and time
will not be accepted by the Siletz Tribal Housing Department.
Qualified applicants will be placed in the lottery drawing, which will be held dur-
ing August on a date approved by the Tribal Council.

What to expect when you’re approved for
hearing aids through Contract Health Services
You will be referred to Costco for hearing aids. Costco hearing aids cost 50-60
percent less than other providers with equal quality.
Your CHS team will help you obtain a Costco membership and help with
transportation barriers. We look forward to working with you.

Tribal Chairman Delores Pigsley presents a $50,000 check to Chief RC Mock of
the Siletz Valley Fire District during a Tribal Council meeting on May 17. The
money will be added to the district’s general fund.
Siletz Community Dental Clinic
Contact the Siletz Community Dental Clinic if you experience dental pain or a
dental emergency. Staff will do everything they can to see you as soon as possible.
Morning check-in time is Monday-Thursday from 8:15-8:45 a.m. and Friday
from 9-9:30 a.m. Afternoon check-in time is Monday-Friday from 1:30-2 p.m.
Harm Reduction
Outreach workers will be in the community working
with individuals most at risk for HIV infection.
Testing. 6 out of 10 Oregonians do not know
their HIV status.

Prevention. There are effective prevention
methods like condoms, refraining from needle
sharing, and medication.
Referral to treatment. Those who test positive
for HIV will be referred to providers to receive

There are many preventative tools you can use to
protect yourself.
Use barrier methods like condoms when
engaging in sexual activity.
If you're at a high risk for HIV infection, there is
medication that can help reduce your risk. Pre-
Exposure Prophylaxis, PrEP, can be prescribed
by a medical provider.
Don't engage in needle sharing.
Provider Collaboration
We will work with healthcare providers to improve
availability of HIV and other STI testing.
We will educate providers to increase their
knowledge of emerging preventative methods.
We will help to connect at risk individuals to the
healthcare system.
