Tribe contracts with willamette Family for A&D services in eugene area
In an effort to provide more compre-
hensive A&D treatment services, Siletz
Tribal Health is contracting with Willa-
mette Family, Inc. We are excited about
the wide range of services that Willamette
Family is able to provide.
Individuals who need outpatient ser-
vices can access weekly groups and indi-
vidual counseling as well as peer support.
Those individuals who need a higher
level of care can participate in intensive
outpatient services that include at least
nine hours of group counseling per week.
Intensive outpatient services can be a
good alternative to residential treatment
for people who have family and other
community support for recovery.
Willamette Family has a women’s spe-
cific program in addition to its downtown
location. Other services provided by Willa-
mette Family are detoxification, including
opiate detox, and residential treatment.
One of Willamette Family’s recovery
support staff is Jeramie Martin, a Siletz
Tribal member.
Willamette Family has long served
American Indians from Oregon Tribes and
the staff is excited about becoming more
of a resource for Siletz Tribal members.
Under this contract, A&D services
at Willamette Family will be provided
at no cost to Siletz Tribal members. For
those people who do not have transpor-
tation to appointments, Lou Carey or
Nora Williams at the Eugene Area Office
(541-484-4234) are available to help with
bus passes.

Tribal members eligible for alternative health care benefits with doctor’s OK
Acupuncture, chiropractic care and
massage therapy are alternative health
choices widely recognized as safe, drug-
free, non-invasive therapies to treat a wide
array of health problems. The Siletz Tribal
Council has appropriated gaming revenue
to pay for these services for Tribal mem-
bers, regardless of residency.
You do not have to be CHS-eligible
to qualify for this benefit. Criteria are that
you be an enrolled Siletz Tribal member.
The process to get approval is as follows:

Visit your primary health care provider
and discuss your medical concerns. Ask
whether acupuncture, chiropractic or
massage therapy services may alleviate
your symptoms.
If your primary care provider believes
acupuncture, chiropractic or massage
therapy services are appropriate, call
CHS.

CHS will approve up to three sessions
and will pay no more than $50 per
session. Patients are responsible for
any costs that exceed $50.

A written letter will be sent to the
patient to take to the acupunctur-
ist/chiropractor/massage therapist
informing him or her of the number of
sessions approved, amount approved
and billing process. If you have insur-
ance, the provider should bill that
insurance first.

Tribal members who live in the
11-county service area and are more
than 40 miles from the Siletz Community
Health Clinic (SCHC) should have their
maintenance medication mailed to them
through SCHC. Not all medications are

in the SCHC formulary, so that is when
you would use your pharmacy card at a
retail pharmacy such as Walgreens, Fred
Meyer or Rite Aid.
Members who live outside of the
11-county service area (Direct Care)
cannot get medication mailed through
the Siletz Clinic, but can get medication
directly through Pequot Pharmacy. Any

Care patients should be mailed to Pequot
directly, which will stretch your card dollars.
Patients who have primary pharmacy
insurance, such as Blue Cross Blue Shield,
AETNA or Medicare Part D, and have to
pay a co-pay up front to the retail pharmacy
can be reimbursed up to the $500 per year.
Members who pay up front for medication
and still have a balance on their pharmacy
card can be reimbursed as well.
To receive a reimbursement, you must
provide the receipt from the pharmacy,
not the cash register receipt. The receipt
must have your name, doctor’s name, date
of service, medication name, medica-
tion dosage and amount paid. Mail your
receipt to Contract Health Services, P.O.
Box 320, Siletz, OR 97380.
Your pharmacy card should not have
your Social Security number on it. If it
does, you need to contact Rhonda Attridge
with Contract Health at 800-628-5720 or
541-444-1236 to get a new card. You will
need to complete an individual registration
form in order to receive your new card.

Council approves health benefits for
members who live out of service area
For time period of July 1, 2012, through June 30, 2013
The Tribal Council has approved the following plan for using the 2013 out-of-area
health benefits.
To qualify, Tribal members must be registered for health care with the Siletz
Tribal Health Department and reside outside of the Tribe’s 11-county service area.
These funds are intended for Tribal members who are not eligible for Contract Health
Services (CHS).
One of the following benefits is available:
$400 – Vision
$1,200 – Dental
$2,000 – Hearing
$1,000 – Medical
Plus $500 maximum pharmacy through Pequot Rx
Prior authorization by CHS is required and benefits must be used within 90 days.
CHS must receive claims by the end of the 90-day funding period. Any funds not used
within 90 days or claims not received will be returned to the pool for redistribution.
An individual can only access benefits one time per fiscal year.
When you call for pre-authorization, be aware that CHS will authorize for
household members only. Authorization will not be granted for anyone not residing
in your household.
Voice mail messages do not hold funds; you must speak to a live person.
