Dear Siletz Tribal Voting Member:
Application for names to be placed on the 2012 ballot for candidates
in the Siletz Tribal Council election
The Election Board apologizes for the recently mailed letter outlining incor­
rect dates for updating your signature card. The Siletz Tribal Council approved
a new signature card and revisions to the Election Ordinance. The significant
change includes new verification options which can reduce the involvement of
the Enrollment Department in the signature verification process.

The new verification process provides Tribal members the opportunity to have
their signature notarized or provide a copy of current identification with name,
address and signature to verify the signature card. If neither of Bose options is

available, the Enrollment Department will still assist in verifying signatures with
documentation on file.

I understand I must be an enrolled member of the Confederated Tribes of
Siletz Indians of Oregon and 18 years of age or older. This application must be
filed with the Election Board by 4 p.m. on Dec. 21, 2011.1 also understand that
if for any reason I decide to withdraw my application for Siletz Tribal Council,
I must withdraw in writing.
If you have a current signature card on file, you are still eligible to vote - you
do not have to take any action.

The revised Election Ordinance is available online at ctsi.nsn.us, select Gov­
ernment Operations then Tribal Ordinances. A new signature card is posted on
the Election Board website under “Members Only.”
If you choose to update your signature card, you may choose the option that
is best for you. Please complete and return to the Election Board by Dec. 30,
2011, to ensure your current signature is verified in time for the 2012 Election.

Voter’s Pamphlet & Statement
If you have any questions, please contact the Election Board at 800-922-1399,
ext. 1256, or 541-444-8256; or elections@ctsi.nsn.us.
If you would like your candidate’s statement and photo of yourself to appear
in the voter’s pamphlet, please submit your statement and recent 3x5 photograph
along with your application. Deadline for statements and photos is Dec. 21,2011,
at 4 p.m. Candidate statements must be no longer than 750 words and must not
be derogatory or contain personal attacks on any one individual. Statements and
photos will appear in the voter’s pamphlet. Photos will be included on the ballot.

The candidate is responsible to ensure receipt of application and statement
; by the deadline. Letters will be sent to all candidates after review and certifi­
