Which Siletz Tribes/Bands Do You Represent?
For increased understanding of Siietz Tribal history, genealogy, and cultural
influences, the Enrollment Department is seeking additional information on which
Siletz ancestral tribes and bands our membership represents as individuals
and families.
This will enable tribal programs and tribal government to more accurately
state important statistical facts about our confederation. It’s also important for
our membership to maintain connections to and reflect pride in all their Siletz
Tribal ancestry.
Please mark all Siletz Tribal bands that you descend from on the form below
and also your family tree information (at least the furthest generation from the
present) you can, then return the completed form to the Enrollment Department,
CTSI, P.O. Box 549, Siletz, OR 97380-0549.
