CHS Current Priority
(Funding) Level
Mission Statement: To ensure
services are available at the end of the
calendar year, CHS (Contract Health
Services) manages money by
fluctuating services to stay within the
budgeted level of funds. You may
experience your care being deferred due
the level of spending occurring in CHS.
Priority I: Emergent/Acutely
Urgent Care Services
Definition:
Diagnostic
or
therapeutic services that are necessary
to prevent the immediate death or
serious impairment of the health of the
individual and which, because of the
threat to the life or health of the
individual, necessitate the use of the
most accessible health care available
and capable of furnishing such service.
Diagnosis and treatment of injuries or
medical conditions that, if left
untreated, would result in uncertain but
potentially grave outcomes.
Priority II: Preventive
Care Services
Definition: Primary health care that
is aimed at the prevention of disease or
disability. This includes services proven
effective in avoiding the occurrence
(primary prevention) of a disease or
disability and services proven effective
in mitigating the consequences of an
illness or condition (secondary
prevention). Level II services are
available at most IHS or tribal facilities.
Priority III: Primary and
Secondary Care Services
Definition: Inpatient and outpatient
care services that involve the treatment
of prevalent illnesses or conditions that
have a significant impact on morbidity
and mortality. This involves treatment

for conditions that may be delayed
without progressive loss of function or
risk of life, limb, or senses. It includes
services that may not be available at
many IHS facilities and/or may require
specialty consultation.
Priority IV: Chronic
Tertiary and Extended
Care Services
Definition: Inpatient and outpatient
care services that (1) are not essential
for initial/emergent diagnosis or
therapy, (2) have less impact on mortality
than morbidity, or (3) are high cost, are
elective, and often require tertiary care
facilities. These services are not readily
available from direct care IHS or tribal
facilities. Careful case management by
the CHS Gatekeeping Review Commit­
tee is a requirement, as is monitoring
by the medical officer or his/her
designee. Depending on cost, the
referral may require concurrence by the
clinical/medical director.
The Siletz CHS Program cur­
rently is approving services at
Level II.
CHS Limits Services
Until Jan. 1,2004
Effective Aug. 1,2003, CHS limits
dental exams for relief of pain only.
Visits will be approved on a case-by-
case basis, the 40-mile restriction still
applies. Appointments may still be
scheduled at Siletz, Grand Ronde, or
Chemawa clinics for dental treatment.
Vision exams and dispensing of
glasses and contacts are being deferred
until Jan. 1, 2004. This service is
available at the Siletz, Grand Ronde,
and Chemawa clinics.
Note: “Open Until Filled”
vacancies may close at any time.
The tribe’s Indian Preference
policy will apply. Tribal government
will not discriminate in selection
because of race, creed, age, sex,
color, national origin, physical
handicap, marital status, politics,
membership or non-membership in
an employee organization.

Assistance
Program Expands
Siletz Warehouse
Beginning Oct. 1,2003, the income
limits for the CTSI Child Care
Assistance Program will increase to 85
percent of the median income for the
state of Oregon. This increase in the
income limit should result in more
families being eligible to receive
assistance for their child care.
To qualify for this program,
families must reside within the
11-county service area and must be
working, going to school, or attending
training or wellness activities, and their
children must be tribal members.
Eligibility for assistance begins on the
day we receive your application, which
must include income verification.

Join Spokesperson Training
Tribal Court Spokesperson training
will be held on two weekends this month.
The first training is Oct. 10-11 and
the second is Oct. 17-18. Applicants can
take advantage of this year’s spokes­
person training, provided in part by
Siletz Tribal Court judges.
Tribal Court’s grant project
coordinator recruited a list of
applicants. Currently, there are nine
enlisted spokespersons. These
individuals are asked to serve only a
minimum of six hours per year,
depending on the case represented.
Applications are accepted
continuously at the Siletz Tribal Court
office. For participation in the training
provided by our grant, the application
deadline was Sept. 30,2003. The chief
judge will determine trainees from their
applications received by the deadline.
The training will be videotaped and any
approved applicants who missed this
training can view it by appointment.
Tribal Court encourages all
interested Siletz Tribal members or
other federally recognized tribal
members living on Siletz reservation
lands to apply.

Home Improvement Loans Available
The Siletz Tribe Revolving Credit
Program currently offers loans to tribal
members for home improvements.
Tribal members can use these loans
for such things as a new roof, new
siding, new carpeting, room additions,
a new furnace, and new appliances.
