This service will not deal with criminal matters. If you have legal concerns, please call us. Don’t delay - we can help most if we can discus your problems at the very begining. If you wait until the last minute, it may be to late for us to help you. Call at any time, including evenings and weekends, to leave us a message. We will call you back as soon as possible. Call anytime to leave a message: 1-800-222-3884 (541)265-5305 By: Howard Roy If you have not received an award letter -- it is not official, we are in the preliminary stages of evaluating your application and what services you can receive from the HIP Program. An award letter will state the category of service that you are approved for. Honesty is always the best policy, false or inaccurate information on your HIP application may affect your eligibility and can lead to other action at any time during the process. If you do not understand a question be sure and ask the Housing Improvement Secretary or Coordinator. You must fill out a new housing application every year in order to be eligible for service. The deadline for FY 99 Housing Improvement Program Applications is December 1,1998. Applications received after December 1,1998 will not be considered for FY99 Housing Improvement Program funds. Anyone requesting Indian Health Service, Housing Improvement or NAHASDA repair assistance is advised to send a copy of your warranty deed or rental agreement with your application in order to shorten your application process time. We encourage clients to ask for a 1 lousing Improvement Program application if you live outside the service area and are interested in NAHASDA repair assistance in order to let us know you are out there. The logistics are not complete on our construction processes. This is the first time the Siletz Tribe has had a Housing Program that would allow the Siletz Tribal members housing repair needs outside the 11 -county service area to be addressed. The application will be mcoiporated into the current Housing Improvement Program Application. We are not sure of the allotted Congress funding for FY 99. Please do not wait until the last minute to apply for Housing Improvement Program or NAHASDA service. Fill out your application today. The Indian Heath Service supplies water and sewer disposal systems in the 11 -county service area of Oregon. However in some cases we have been able to provide service in other areas of Oregon with the assistance of other Oregon Tribes and I.H.S. working cooperatively. Who qualifies for the program? Any Siletz Tribal Member is eligible. First, you must obtain an I.H.S. application from one of the area offices or the HIP office in Siletz. Fill out, sign and send it to the HIP office in Siletz. Septic Systems that are spilling effluent onto the ground present a health hazard and should be reported to the HIP Program for site evaluation from I.H.S. HIP will provide the necessary personnel deployment and procedures to begin the health threat remedy process. This type of situation is definite priority for service. We also serve new residential home sites. It is important to get your application request to us as soon as possible for scheduling purposes. There is a waiting list and new septic systems can only be installed between the months of May through October. The sooner you let us know about your problem or upcoming project the easier it is to maintain and schedule service for everyone. We are planning for the upcoming year’s HIP program and are looking for suggestions and specific needs as we try to meet our mission statement for the Tribal Members. The purpose of HIP is to provide grant assistance to Tribal members to improve their housing situations and to give them the opportunity for a safe, decent home and a suitable living environment, and to prescribe the terms and conditions under which assistance is given to Siletz Indians under the Housing Improvement Program (HIP).
