M ajor Duties: Conducts home visits within Portland area for enrolled
Head Start families. Ensures that needed social services are provided to all
families within Head Start as identified in Family Needs Assessment
(FNA). Empower families with individual problem solving and /or
counseling, and long- and short-term goal setting. Assists families by
providing direct services, by making necessary referrals, or by-advocacy.
Responsible for health requirements of all families and children within
Head Start to include: (1) nutrition assessment and feedback, (2) physical
examination and follow-up, (3) dental examination, treatment and follow
up, (4) immunizations and follow-up, (5) health histones, and others, as
assigned. Maintains individual health records, immunization, and
tracking system for all enrolled children. Participates in classroom

activities as scheduled. Assists in maintaining directory of health & social
service resources, and develops a system to use these resources.
Coordinates with Head Start staff and facilitates weekly meetings
ensuring each family is reviewed with appropriate follow-up completed
and documented. Maintains complete and accurate required family/child
files, forms, and records to document and track needs of families and
services provided. Participates in the conduct and analysis of the annual
community needs assessment. Works with staff in the identification,
reporting, and provision of supportive services in child abuse cases in
coordination with applicable agencies. Based on results of the FNA, assist
in scheduling of parent training. Consult with Family Services
Coordinator on regular basis.

Scholarship award amount will be set annually. Scholarship may not be awarded
