The VSM committee has agreeu to proviue a set of guiuing principles that
help ensure
the sanitation anu safety of the whole
Participant Standards
hll youth unuer the age of 10 must be with an oluer sibling, parent or
care proviuer (ORS 163.545)
hny youth, unuer 10, without a chaperone WILL be feu anu sent
home if they cannot proviue name of oluer care proviuer
hll participants WILL have shoes anu shirts ( T-shirts available to
borrow)

By participating or allowing the participation by my child(ren) I:
HEREBY RELEhSE the Vernonia Summer Meals Program, Vernonia School District, or
its auministrators, volunteers or employees, from all liability, claims, uemanus, losses or
uamages on my account anu on account of my chilu(ren); anu if, uespite this release anu
waiver of liability, assumption of risk, anu inuemnity agreement, I, or anyone on my behalf,
makes a claim against the Committee or its agents, I WILL INDEMNIFY, ShVE hND
HOLD THEM HhRMLESS from any litigation expenses, attorney fees, loss, liability, uam-
age, or cost which may result from such claim, to the fullest extent permitteu by law.
