The school nurse provides leadership for the provision of health services
in the La Grande School District. As the health care expert within the
school, the school nurse assesses the overall system of care and develops a
plan for individual students and the district for ensuring that health needs
are met.
Youth Transition Program (YTP)
Our Youth Transition Program is designed to help high school students
in our Special Education Program access support in entering college or

community and state services like Vocational Rehabilitation. Additionally,
they support by helping families complete paperwork for Social Security,
Oregon Health Plan, Disability Services, and many more. We want all La

support them in transitioning to adulthood.
