As soon as funds are made avail
able, Douglas county Is to Inaugurate
a complete and full time health unit
under the Rockefeller foundation plan.
Thta health unit consists of a physi
cian, and two health nurses, and a
enographer or clerk, who will devote
tie full time to the work of protec

Ibis unit Is made possible through
the Rockefeller foundation, a fund
aside for the purpose of providing
suitable ' organizations to safeguard
and nrotect the health of the public.
To provide funds for this work the j
county appropriates me sum or
14,420. The yearly budget for the unit
la $10,000 and the balance of this
fund Is made up from the state board
of health, the Rockefeller Foundation
and the IT. S. department of labor.
The detailed work of the health unit
is given In the following outline .and
plan of cooperative work, as present
ed In the health bulletin submitted to
the county by the state board of
health.
The Oregon state board of health is
willing to enter into a cooperative
program with Douglas county for the
purpose of establishing a county
health department to demonstrate to
the pooplo of the county the advan
tages of and tho necessity of such an
organization for the protection of the
public health.
The following units of work shall
constitute the foundation upon which
all the activities of the department
shall be based:
1. Unit of quarantine and epidemi
ology. 2. Unit of health education.
3. Unit of sanitation.
4. Tuberculosis unit.
5. Infant hygiene unit.
6. Unit of physical inspection of
school children.
7. Dispensary unit
1. Unit of guarantlne and epidemi
ology: The work of this unit .shall be
conducted in accordance with the
laws of the elate and the rules and
regulations of the state board of
health, which laws and regulations
are to be regarded as the minimum
requirements. The health officer shall
Conduct such other epidemicio-logical
work as his time will permit, that
may be of value in protecting the
public health.
IL Unit of public health education:
The object of this unit is to properly
Inform the peonle of the county upon
matter relating to the public health.
In the conduction of this unit free
use should be made of the county
press for the publication of suitable
articles. Personal and circular let
ters and personal visits uud confer
ences, together with the dtiirlbution
of pamphlets, folders, bulletins, and
posters should be freely indulged in.
Exhibits at county and community
fairs will bo helpful.
The most effoctive means of edu
cating the people is by the use of lec
tures Illustrated with lantern elides.
These lectures should be carefully
prepared and should bo given by tho
health officer at every school district
hi the county.
The accomplishments of the de
partment, ;nt the end Vf any ' given
period, will be commensurate with
tho amount and effectiveness of tho
educational propaganda tho depart
ment has or has not conducted. It is
Important therefore that this unit of
work be vigorously pursued at all
times. ... , .
III. Unit of sanitation: Sanitation
means the establishment of conditions
favorable to health. ' This unit of
work, however, should be specifically
Interpieted to mean the following:
1. A survey of the homes In the
county, to determine the following:
(a) Presence of latrine, (b) Type of
iRtrine. (c) Location of latrine and
relation to water supply, (d) Type of
wnter supply, (e) Drainage.
2. A definite attempt to have reme
died any conditions fould that are po
tential dangers to the health of the
individuals occupying the premises.
3. The Inspection of all public
places to see that latrines and water
supplies are in such conditions as
not to be a source of danger to those
who frequent the place, or to those in
the adjacent community.
4. The Inspection of all dairies in
the county to see that the milk is pro
duced under sanitary conditions and
comes from a herd free from tuber
culosis. 6. Tho prompt attention to all com:
plaints relative to Insanitary condi
tions. 6. Tho abatement of insanitary
nuisances.
In teaching sanitation we, are
simply teaching cleanliness and it la
well lo keep In mind that there are
two kinds of cleanliness; physical or
esthetic, and biological cleanliness.
The difference between the two
may be illustrated by a comparison
between the dangers from accumula
ted refuse on the school grounds and
the use of a common drinking cup at.
the school. If we remove the accumu
lated rubbish what have we accom
plished towards the prevention of di
sease? Again, an etanus spore on the
shining blade of the surgeon's knife
makes that instrument filthy, where
as many such spores upon the breast
of a chick may be harmless when in
jected. ,
IV. Tuberculosis unit: In the con
duction of this unit the following
outline should be followed:

ing occurred In the county during the
two previous years.
2. Make a record of all premature
and still births having occurred in

years.
3. Make a record of all breast fed
children.
4. Make a record of all bottle fed
children. . '
5. Make a record of all children In
the county under two years of age
suffering from digestive disturbances
or diarrheal diseases.
6'. Make a record of all midwives
who practice in the county.
7. . Muke a tack map Illustrating
items 1 to 6 inclusive. '
8. Visit all homes represented by
2, 4, and 6.
9. If the midwives cannot be elim
inated from the practice of midwifery
then Instruct them in the rudiments
of asepsis and obstetrics.
10. Have all local registrars report
weekly all births having occurred In
their districts.
11. Request the physicians and mid
wives to report their pre-natal cases
as soon as they come under their ob
servation. 12. Visit all homes represented by
items 10 and 11.
13. Conduct In convenient locations
In tho county . baby clinic!! for the
weighing and measuring of the babies
of the community, and lor physical
examinations to detect correctable de
fects. These clinics should also be
prenatal clinics where expectant
mothers may come for examination
and advice.
VI. Physical inspection of Schools
and school children:
1. A physical record should be
made out for each pupil upon his en
trance to school. This should follow
him throughout , his school life and
contain his complete physical history.
2. Systematic physical examination
should be given to all school children
inviting neighborhood physlciuns to
assist.
3. ParentB or guardians should be
confidentially notified of the physical
condition with the suggestion that the
child be sent to the family physician
or denist.
4. Intensive and individual follow
up work should be done to insure the
correction of as many defects as pos
sible; the object df the unit not oe-
the examination but the correc
tion of the defects.
6 An effort should be made to es
tablish clinics at some suitable place,
for the correction of defects in In
digent cases. '

schools for the detection and preven
tion of spread of communicable rti
seases. A safe water supply, Individ
ual drinking cups and towels, improv
ed privies of an approved type, proper
ventilation. lighting, and heating

Before a dispensary Is held, It
should be thoroughly advertised at
least one week In advance.
The state agrees to furnish such
vaccines as may bo needed at special
ly quoted prices.
Method of procedure: In the execu
tion of the above outlined units of
work, it Is believed that the best re
sults will be obtained by pursuing a
modified, intensive method.
Of the seven units of work all re
late to public health hazards that are
constantly present, and will require
attention at all times. Still It Is pos
sible and desirable to intensify the
work of each unit in rotation and at
the same time not neglect the other
units. Kqr instance, with the begin
ning of spring, dovote particular time
eflorts to the unit of Infant hygiene.
Endeavor to arouse greater Interest
of the mothers and others In the chil
dren. Put on a special publicity cam
paign and concentrate the efforts of
the department on this particular unit
at this particular time, and at the
same time carry on in a less Intensive j
manner the other units of work. In I
the fall, Intensify the tuberculosis!
unit and conduct In a less Intenslvo
I way the other units, and so on
through the entire program. To In
sure the maximum results each unit
